Primary Care Physician – PACE Program | Relationship-Focused, Mission-Driven Care

As part of a mission-driven interdisciplinary team, our Primary Care Physicians build long-term relationships with participants and see the impact of their care every day.
This value-based model allows you to deliver comprehensive, holistic care—without the pressure of high patient volume.
What We Offer:
- Compensation: $315,900 – $386,600 annually + quarterly bonus + equity
- Schedule: Monday–Friday, 8am–5pm (no hospital rounds)
- Workload: Average of 8 patients/day
- PTO: 17 days + 6 sick days + 12 holidays

Key Responsibilities:
- Perform comprehensive assessments and lead interdisciplinary care planning
- Coordinate care across settings (acute, short-term, long-term)
- Conduct occasional home visits
- Provide compassionate, holistic care focused on improving quality of life
Job Requirements:
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Doctorate Degree
- Board Certification – Internal Medicine or Family Practice or Geriatrics (preferred)
- Active California Physician License without Restrictions
- Active state CDS Certification and DEA
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
