Job Title: Field Care Manager, Behavioral Health
Location: Remote (Home Office) with 15–20% travel to Wayne or Macomb County, Michigan

Position Overview
Join our compassionate team and help us put health first. As a Field Care Manager, Behavioral Health , you will assess and support members to achieve optimal wellness by connecting them with appropriate resources. This role involves varied assignments and requires independent judgment.
You will work from a home office and travel locally (15–20%) to conduct in-home visits with Medicare/Medicaid members in Wayne or Macomb County, Michigan.
Key Responsibilities
- Manage a caseload and conduct assessments in-home, community-based settings, and telephonically.
- Collaborate with the Interdisciplinary Care Team (ICT) to support members with complex medical needs.
- Develop and coordinate care plans, address barriers to care, and advocate for member outcomes.
- Complete medical attestations and clinical oversight reviews.
- Ensure members receive services in the least restrictive setting.
- Modify Individual Care Plans and involve relevant care team members.
- Serve as the primary contact for the ICT and coordinate with internal and external resources.
Required Qualifications
- Licensed Bachelor's-level Social Worker or active limited license with intent to obtain full licensure.
- Minimum 2 years of post-degree clinical experience in behavioral health.
- Michigan state licensure in field of study.
- Must reside in Michigan and be able to travel to Wayne or Macomb Counties.
- Experience with adult populations and disease management.
- Knowledge of community health and social service agencies.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficiency with electronic medical records and Microsoft Office (Word, Outlook, Excel).
- Must work full-time (40 hours/week), Monday–Friday.
- Valid driver’s license, auto insurance (minimum 100/300/100 coverage), and access to a vehicle.
- Dedicated home office space with a locked door for privacy.
- High-speed internet (DSL or cable; satellite not permitted).
- Must participate in Humana/Senior Bridge’s TB screening program.
Preferred Qualifications
- Licensed Master's-level Social Worker (LMSW/LCSW)
- Experience with in-home assessments and care coordination.
- Background in health promotion, coaching, and wellness.
- Experience with Medicaid Long Term Care and managed care.
- Bilingual in Spanish, Arabic, or Chaldean Neo-Aramaic.
- Certification or knowledge in Motivational Interviewing.
