Job Description

We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to provide high-quality social work services to individuals and families. The LMSW will assess client needs, develop care plans, and collaborate with interdisciplinary teams to ensure effective service delivery and positive client outcomes.

Job Responsibilities

• Conduct comprehensive psychosocial assessments to evaluate client needs, strengths, and challenges
• Develop, implement, and monitor individualized care or service plans
• Provide case management, counseling, and supportive services to clients and families
• Advocate for clients by coordinating community resources and social services
• Maintain accurate, timely, and confidential clinical documentation in compliance with agency, state, and federal regulations
• Collaborate with multidisciplinary teams, including healthcare providers, educators, and community partners
• Monitor client progress and adjust interventions as needed
• Provide crisis intervention and referrals when appropriate
• Ensure compliance with ethical standards and professional social work practices
• Participate in supervision, team meetings, and ongoing professional development activities

Required Skills & Experience

• Minimum of one (1) year of LMSW experience

Education

• Master’s Degree in Social Work (MSW)

Certifications & Licensure

• Valid Licensed Master of Social Work (LMSW) license issued by NYSED
