Overview

Join Nurchure Staffing Solutions’ dedicated Home Health team as a Physical Therapist , delivering exceptional, compassionate care directly to patients in their homes. In this vital role, you will assess patient needs, develop personalized treatment plans, and empower individuals to regain mobility, independence, and confidence.

Key Responsibilities

• Perform comprehensive physical therapy assessments and create individualized treatment programs.
• Implement therapeutic exercises and interventions to promote mobility, strength, and function.
• Monitor, document, and report patient progress and response to therapy.
• Educate patients and caregivers on exercise programs, mobility aids, and safe home practices.
• Supervise and provide guidance to Physical Therapy Assistants and Certified Nursing Assistants.
• Maintain accurate and timely clinical documentation in compliance with healthcare regulations.
• Collaborate with interdisciplinary teams to optimize patient care outcomes.

Schedule & Shift Details

• Monday–Friday, day shift (start and end times vary by patient schedule)
• One weekend every 6 weeks (Saturday and Sunday day shift)
• Two holidays per year (one summer, one winter rotation)
• No on-call requirements

Travel Requirements

• Local travel to patient residences within the Springfield, IL area
• Must have a valid driver’s license and reliable transportation

Qualifications

Required:

• Master’s degree or higher in Physical Therapy from an accredited institution
• Valid Physical Therapist (PT) license in the State of Illinois
• Minimum 1 year of experience in a healthcare setting
• Valid driver’s license and dependable vehicle

Preferred:

• Please include your Illinois PT license number on your application
